Most keratoconus now adays can be treated without corneal transplants. We generally do a corneal strengthening procedure called collagen cross linking followed by corneal or scleral contact lenses for visual rehabilitation.
In advanced cases we may need to do deep anterior lamellar keratoplasty which is advanced corneal transplant where patients most posterior corneal layer i.e. endothelium is preserved to reduce graft rejection chances and better graft survival rates.
Yes, in approximately 90 % cases it is bilateral, however onset, grade of severity & rate of progression could be asymmetrical.
